How
To Request a Keralac Nail Gel Consult:

First read
about Keralac Nail Gel and learn more about its indications and contraindications
and read and agree to the Waiver of Liability.

Next, accurately
and truthfully complete and submit a medical consultation questionnaire

Your medial
consultation will be reviewed by one of our Physicians and if approved
you will receive a Keralac Nail Gel prescription with 6 refills which
can be filled at any U.S. pharmacy of your choice. No medication purchase
required- you can choose any pharmacy where your medical insurance accepted.

This physician
evaluation is RISK-FREE. If Keralac Nail Gel prescription is
not appropriate for you there is No charge. The initial consultation
fee for the physician evaluation is only $49.95.
CONTRAINDICATIONS
Keralac Nail
Gel is a NEW prescription site-specific tissue softener that can be used
alone or as concomitant therapy for thickened skin and thickened
nails due to psoriasis or mild to moderate nail fungal infections
in immunocompetent individuals. Keralac Nail Gel is CONTRAINDICATED
and must NOT be used in anyone with the following conditions:

Do NOT user
Keralac Nail Gel if you are pregnant, attempting to become pregnant,
or at a high risk of becoming pregnant. Keralac Nail Gel (50% urea)
is a Pregnancy Category B which means that there are no adequate studies
of Keralac Nail Gel in animals or humans. Therefore Keralac
Nail Gel may pose potential risk to the fetus and should not be used
by pregnant women.

Do NOT use
Keralac Nail Gel if you are breast feeding.

Do NOT use
Keralac Nail Gel if you are allergic or sensitive to any of the ingredients:
50% urea, lactic acid, zinc, PEG-6, Propylene Glycol, Trolamine, Xanthan
Gum, Edetate Disodium, Hydroxyethylcellulose.

Keralac Nail
Gel is contraindicated if you have diabetes, diabetic neuropathy since
small infections or tiny lacerations can become major infections and result
in loss of limb. All treatment in diabetics should be done my a medical
professional.

Keralac Nail
Gel is contraindicated if you have severe case of nail fungus infection
involving many nails and involving the entire nail to the cuticle with
the nail crumbling and lifting from the nail bed accompanied by pain.

We do NOT prescribe
Keralac Nail Gel to anyone under the age of 18

Keralac Nail
Gel is ONLY for use on toe and finger nails and immediately adjacent skin
and thickened skin areas. Do NOT use in eyes, orally, on mucous membranes
or in vaginal area.

Keralac Nail
Gel is for EXTERNAL use only! Avoid contact with wyes, lips or mucous
membranes.
IMPORTANT
PRECAUTIONS and WARNINGS
-
Keralac Nail
Gel is for EXTERNAL USE ONLY. Avoid getting Keralac Nail Gel
in your eyes, nose or mouth. Flush with water and seek immediate
medical attention!

Apply Keralac
Nail Gel only to the nails and immediately adjacent skin and thickened
skin areas. Do not apply intravaginally or to mucous membranes or
lips.
-
Do NOT use
nail polish, artificial nails, or any other cosmetic nail product on the
nails with Keralac Nail Gel.
-
For thickened
nails due to nail fungal infection, it is recommended that Keralac Nail
Gel be used along with an antifungal agent such as Penlac
Nail Lacquer.
-
If the area
of application shows signs of increased irritation such as increased redness,
swelling, burning, blistering, or oozing, DISCONTINUE USE and seek immediate
medical attention from your regular physician. Keralac Nail Gel
is to be used as part of a comprehensive skin and nail care program.
In some cases regular periodic trimming and filing of infected nail
by a professional may be necessary in addition to your own regular nail
care.

PATIENT INSTRUCTIONS
HOW TO APPLY Keralac
Nail GEL - Easy to Use:
-
Step 1: Before
starting treatment remove any loose nail or nail material using nail clippers
or nail files (emery board).

Step 2: Apply
a single, even coat of gel directly to the problem area once or twice a
day. Apply evenly over the entire nail. Where possible,
apply to the underside of the nail and to the skin beneath the nail.

If using Keralac
Nail Gel concomitantly with other topical antifungal or steroid therapies,
apply Keralac Nail Gel first. Allow gel to completely dry before
apply other topicals on top of gel.

Allow gel to
completely dry before putting on socks or stockings.
-
Apply Keralac
Nail gel daily to affected skin and nails over the previous coats..
-
Always close
bottle tightly and protect from light by placing bottle into carton after
each use.

It's WORTH
THE WAIT: It may take several months to start to see improvement.
ADVERSE REACTIONS
- SIDE EFFECTS:
-
The majority
of patients using Keralac Nail Gel experience no side effects and side
effects are mild if any. The side effects when used topically as
directed are limited to transient stinging, burning, itching or irritation
at site of application which disappears on discontinuing the medication.
If you experience increasing redness, swelling, pain.... stop use
of Keralac Nail gel and contact your doctor.

If you get
the medication in your eyes, rinse immediately with water and contact your
regular physician!

There are NO
systemic effects when used appropriately

I DO NOT HAVE ANY OF THE
ABOVE CONTRAINDICATIONS:
By accepting "I Agree" means that
you have read the above contraindication, precautions and warnings and
honestly acknowledge that you do not have any of the above contraindications
to Keralac Nail Gel therapy. Also, that
I am aware of the side effects ! I fully understand that Keralac
Nail Gel is indicated as sole therapy or concomitant therapy for
thickened, hypertrophic skin and nails and only for mild to moderate fungal
infection of the nails in immunocompetent individuals. Keralac
Nail gel is not recommended for anyone with diabetic foot
or skin changes and if used must only be used under DIRECT physician supervision
otherwise loss of limb and severe infection and even death could occur.
I understand that it is solely my responsibility to seek diagnostic and
medical treatment for any underlying medical conditions from my own regular
primary care physician. |
