Before Proceeding to the Medical Consultation Form please read
CONTRAINDICATIONS  and  WARNINGS 
and accept the Waiver of Liability


In order for Medical Wellness Center Physicians to provide you with the best care you need  read and agree to Waiver of  Liability and you need to reply honesty to all questions and you need to understand 
any and all risks and side effects associated with Denavir.



 To order DENAVIR Rx:
  1. First read about Denavir and it's contraindications, and read and agree to the Waiver of Liability
  2. Next, accurately and truthfully complete and submit a medical consultation questionnaire
  3. Your medial consultation will be reviewed by one of our Physicians and if approved you will receive a Denavir prescription.  This physician evaluation is RISK-FREE.  If Denavir is not appropriate for you there is No charge.  The initial consultation fee for the physician evaluation is only $49.95. 
  4. If you you are experiencing an acute cold sore outbreak and need a prescription within 24 hours, you can indicate this urgency and have your medical consultation reviewed and a prescription called in to a local United States pharmacy of your choice within 24 hours.
CONTRAINDICATIONS:
  • Anyone with any compromised immune system, advanced HIV disease, kidney or organ transplant should not use Denavir because Denavir as only been clinically studies in healthy adults.  If you have any questions about your medical condition, consult your primary care physician before taking Denavir.
  • Hypersensitivity to any ingredient of Denavir: penciclovir or famciclovir or any of the following inactive ingredients: cetomacrogol 1000BP, cetostearyl alcohol, mineral oil, propylene glycol, purified water and white petrolatum.
  • Absolutely no one under the age of 18 should take Denavir.
  • Absolutely no one who has had an organ transplant, a kidney transplant or a bone marrow transplant should take Denavir
  • Absolutely no one with advanced HIV disease should take Denavir.
  • Pregnant women and nursing mothers should NOT take Denavir, there are no adequate studies to guarantee safety, and whether the active ingredients  may get into breast milk.
IMPORTANT WARNING:
Denavir is approved for the treatment of acute outbreaks of cold sores ADULTS with NORMAL immune systems only! There are no adequate studies of Denavir during pregnancy, therefore Denavir should not be used by pregnant women or nursing woman.  Once infected, the herpes virus stays in your body forever.  You are most contagious during active cold sore outbreaks, taking Denavir shortens the duration of the cold sore by at least one-half  but does not necessarily affect your being contagious to others.  Even when you have no active lesions you could still be shedding the virus in your saliva.  To minimize spreading of the virus, refrain from kissing when you have open lesions and minimize touching of lesions.

Denavir should only be applied externally to the lips and face.  Denavir should NEVER be applied to mucous membranes, inside the mouth, or for genital herpes.   Use caution and do not get in eyes.   Denavir is to be used externally, topically only!  If ingested by mistake call 911 immediately or your local poison center for emergency treatment.

IMPORTANT INFORMATION:
Denavir is not a cure for  cold sores and transmission of infection to others can occur during asymptomatic viral shedding even when there are no lesions present. Denavir does not alter the need for precaution in spreading the disease to others.

I DO NOT HAVE ANY OF THE ABOVE CONTRAINDICATIONS:
By accepting "I Agree" means that you have read the above contraindication and warnings and honestly acknowledge that you do not have any of the above contraindications to Denavir therapy.  Also, that I am aware that I may still be infectious even if taking Denavir   or when I am asymptomatic.
